Wound Care

Wound Care

Our registered nurses provide comprehensive wound assessment and management for acute and chronic wounds. This includes post-surgical wounds, pressure injuries, leg ulcers, diabetic foot wounds, and complex dressings. We develop evidence-based wound care plans and monitor healing progress with regular clinical reviews.

Medication Management

Managing complex medication regimens can be challenging. Our nurses provide safe administration of medications, including injections and infusions, along with thorough education for patients and carers. We conduct medication reviews to identify risks and support medication compliance.

Post-Hospital Care

The transition from hospital to home is a critical period. Our nurses provide clinical monitoring and care coordination to support veterans recovering at home. We work closely with hospital discharge teams and GPs to support continuity of care.

Palliative Care

Compassionate end-of-life support at home

Palliative Care

We provide dignified, compassionate palliative nursing care that honours the wishes of veterans and their families. Our nurses work as part of a multidisciplinary palliative care team to manage symptoms and ensure comfort, in accordance with the veteran's care plan.

Continence Management

Discreet, respectful continence assessment and care

Continence Management

Continence issues can significantly impact quality of life. Our nurses conduct thorough assessments and develop personalised management plans that preserve dignity and promote independence. We provide education, product recommendations, and ongoing monitoring.

Allied Health Support

Coordinated access to allied health in the home

Allied Health Support

We coordinate and support access to allied health services delivered in the home, including physiotherapy, occupational therapy, and podiatry, where approved by DVA. Our nurses work collaboratively with allied health professionals to support integrated care.

Clinical Governance

Incident Reporting & Management

SWAST CARE is committed to a transparent, open culture of safety. All incidents, near misses, and adverse events are reported, reviewed, and acted upon to continuously improve the quality and safety of care.

What to Report

Clinical Incidents

Any unplanned event during the delivery of care that resulted in, or had the potential to result in, harm to a patient — including medication errors, falls, wound complications, or equipment failures.

Near Misses

An event that did not cause harm but could have under different circumstances. Near misses are as important to report as actual incidents — they identify risks before harm occurs.

Adverse Events

An unintended injury or complication arising from healthcare management rather than the patient's underlying condition, resulting in disability, prolonged care, or death.

Complaints & Concerns

Any concern raised by a patient, family member, carer, or healthcare professional regarding the quality, safety, or appropriateness of care provided by SWAST CARE.

Our Incident Management Process

Immediate Response

The attending nurse takes immediate action to ensure patient safety, provides first aid or escalates to emergency services where required, and notifies the clinical supervisor.

Incident Documentation

The incident is documented in full within 24 hours of occurrence, including date, time, location, persons involved, clinical details, and immediate actions taken.

Clinical Review

The clinical manager conducts a review of the incident, assesses severity using the Australian Commission on Safety and Quality in Health Care (ACSQHC) framework, and determines whether a root cause analysis is required.

DVA Notification

Where required under the DVA Community Nursing Program contract, SWAST CARE notifies DVA of serious incidents within the timeframes specified in the contract. DVA retains the right to investigate any notifiable incident.

Corrective Action

A corrective action plan is developed and implemented to address identified risks. Actions are monitored for completion and effectiveness, and outcomes are documented.

Feedback & Closure

The patient, family, or referrer is informed of the outcome of the review where appropriate. The incident is formally closed once all corrective actions are verified as complete.

DVA Contractual Obligations

As a DVA contracted provider, SWAST CARE is required to maintain a documented incident management system and report notifiable incidents to DVA in accordance with the Community Nursing Program contract conditions. All incident records are retained for a minimum of seven years and are available for DVA audit upon request.

Healthcare professionals, patients, family members, and carers can report an incident or concern at any time. All reports are treated confidentially and managed in accordance with our Incident Management Policy.

Ready to Refer a Patient?

Our clinical team responds to all referrals within one business day. DVA-funded services are subject to DVA approval and clinical assessment.
