Are You Entitled to Funded Nursing Care?

If you hold a DVA Gold Card or White Card, you may be entitled to community nursing services funded by DVA. SWAST CARE is a DVA contracted provider that works directly with the Department of Veterans' Affairs. All services are subject to DVA approval and clinical assessment.

  • Gold Card holders: access to all clinically required nursing services
  • White Card holders: nursing care for accepted service-related conditions
  • DVA-funded services subject to DVA approval and clinical assessment
  • Referrals accepted from GPs, specialists, and DVA directly

How DVA Referrals Work

Getting started is straightforward. Your GP, specialist, or DVA case manager submits a referral and we coordinate with DVA from there.

Receive a Referral

A GP, specialist, hospital, or DVA case manager submits a referral to SWAST CARE on behalf of the veteran.

Clinical Assessment

Our registered nurse conducts a comprehensive in-home assessment to develop a personalised care plan.

Care Begins

Nursing visits commence at a schedule that suits the veteran — with regular reviews and family communication.
